(A) Comparison on healing effects and cost of the hydrofibre dressing vs the calcium alginate dressing for treatment of pressure ulcers

    The purpose of this study is to compare the pressure ulcer effects, ease of application, dressing wear time and pressure ulcer cost of the hydrofibre dresssing vs the calcium alginate dressing. The subjects were composed of patients suffering from third and fourth pressure ulcer at G university hospital and its adjacent geriatrics hospitals in J city, South Gyeongsang Province. The period ranged from August, to November, 2003. The subjects were divided into a hydrofibre dressing group(10 patients) and a calcium alginate dressing group(7 patients). Data were collected before dressing, one week later, two weeks later and three weeks later. The dependent variables were healing effects of pressure ulcer, ease of application, the dressing wear time, three-week dressing cost and one-day dressing cost. The data were analysed with Kolmogorov-Smirnov, descriptive statistics, t-test, Chi-square, ANCOVA, and Mann-Whitney by SPSS WIN program. The results of the study were summarized as follows: 1) The area of the pressure ulcer in the hydrofibre dressing group decreased by 38.78 percent(one week later), 63.33 percent(two weeks later), and 79.13 percent(three weeks later). the calcium alginate dressing group decreased by 17.93 percent(one week later), 32.42 percent(two weeks later), and 45.10 percent(three weeks later). In the area of pressure ulcers there was no significant difference a week later and two weeks later but there was a marginally significant difference three week later(F=3.14, P=0.098) between the two dressings. This means that the hydorfibre dressing group had better pressure ulcer healing effects than calcium alginate dressing group. 2) In the ease of application the hydrofibre dressing group was better than the calcium alginate dressing group in terms of ease of removal (F=5.20, P=0.02), and duration of each dressing(F=3.34, P=0.089). But there was no significant difference in terms of presence of bleeding, the amount of exudate at the time of removal, presence of maceration and adhesion. 3) In the mean dressing time the hydrofibre dressing group was 3797.14 minutes(63 hours 17 minutes), and that calcium alginate dressing group accounted for 3419.49 minutes(56 hours 59 minutes). The dressing wear time per dressing of the hydrofibre dressing group didn't show any significant difference. 4) Three weeks dressing cost indicates that in terms of the cost of dressing material the hydrofibre dressing group accounted for 3379.30 won, while the calcium alginate dressing group accounted for 2790.86 won, and that in terms of other material cost, the hydrofibre dressing group accounted for 15256.0 won, whereas the calcium alginate dressing group accounted for 21136.14 won, and that in terms of the labor cost of the nurses, the hydrofibre dressing group accounted for 16308.0 won, whereas the calcium alginate dressing group accounted for 19170.0 won and that in terms of the total cost of pressure ulcer, the hydrofibre dressing group accounted for 34943.30 won, whereas the calcium alginate dressing group accounted for 3097.0 won. One-day dressing cost indicates that in terms of the cost of dressing material, the hydrofibre dressing group accounted for at 160.92 won, whereas the calcium alginate dressing group accounted for 132.90 group and that in terms of the cost of other materials, the hydrofibre dressing group accounted for at 726.48 won, whereas the calcium alginate dressing group accounted for 1006.48 won, and that in terms of the labor cost of the nurses, the hydrofibre dressing group accounted for 776.57 won, whereas the calcium alginate dressing group accounted for 912.86 won and that in terms of the total cost of pressure ulcer, the hydrofibre dressing group accounted for 1663.97 won, whereas the calcium alginate dressing group accounted for 2052.24 won. There was no significant difference between the two groups in terms of the cost of dressing material, the cost of other materials, and the labor cost of the nurses for three weeks and for a day. There was a marginally significant difference between the two dressing group in terms of the total cost of pressure ulcer. The result indicates that hydrofibre dressing was more economical than calcium alginate dressing(F=3.335, P=0.089). In conclusion the hydrofibre dressing is more effective and economical than the calcium alginate dressing. Yet, there was no difference in dressing wear time between the two dressing group. Accordingly, hydrofibre dressing can be recommended as effective rather than calcium alginate dressing for treating the third and fourth stage pressure ulcer.
