Comparison of Positive Rate of Urease Test Using One Freeze Dried Specimen with That of Urease Test Using Three Fresh Specimens

Urease test has relatively high sensitivity and specificity and it is easy to use for the diagnosis of Helicobacter pylori infection. In the previous study of GNUH in 1996, the positive rate of urease test using three specimens in one kit(74%) is higher than using one specimen in one kit(49%). But, it is not simple that multiple gastric antral and body biopsies were obtained from a child. The sensitive diagnositic method with less number of specimen is needed. Because freeze dried specimen is easy destroyed and absorbs water, the urease in specimen can easy contact the reagent. In this study, we compared positive rate of urease test using one freeze dried speciemen with using three fresh specimens in same patient. Multiple gastric antral and body biopsies were obtained from 127 patients at the Department of Pediatrics of GNUH, from July 1996 to June 2000. Each one specimen of gastric antrum and body was freeze dried. Each three fresh pieces of antral and body biopsy specimens were innoculated into the buffered phenol red urea broth and incubated at 37℃ for 48 hours to detect the color change. Each one freeze dried piece of antral and body biopsy specimens was tested by the same method. The positive rate of urease test using one freeze dried specimen is higher(72.4%) than that using three fresh specimens(50.4%) in gastric antrum. The positive rate of urease test using one freeze dried specimen(85%) is almost equal to that using three fresh specimens in gastric body(86.6%). Therefore, It is more sensive in the diagnosis of Helicobacter pylori infection and can reduce the time of obtaining the biopsy specimen that the urease test is performed using the freeze dried specimen.
