신생아 중환자실 간호사의 통증관리에 대한 지식과 통증간호 수행 및 장애 요인에 관한 연구
(A) Study on Pain Management Knowledge, Nursing Intervention for pain and Barriers of Pan care of NICU Nurses

This study is a survey research which is tried to understand the knowledge about pain management, pain care intervention and barriers of pain care perceived by the nurse working in the neonatal intensive care unit. The data was collected from January 10th, 2004 to February 28th, 2004. The subjects of 103 nurses were drawn from nurses working at the neonatal intensive care unit in three university hospitals, two general hospitals and two specialized hospitals of pediatrics located in Gyeongsagnamdo and Busan metropolitan city. The tools used in this study were the questionnaire on the subject's general characteristics, pain management knowledge, nursing intervention for pain and barriers of the pain care. The reliabilities of these tools were Guttman Split-half .78 on the pain management knowledge scale, Cronbach's Alpha .67 on nursing intervention for pain scale and Cronbach's Alpha=.69 on barrier scale. The collected data was analyzed into frequency, percentage, mean, standard deviation, Gttman Split-half, and Cronbach's Alpha. The results are summarized as follows: 1) The total mean of pain management knowledge was 29.8(±2.77) of 40 and 74.5% of right answer. 2) The degree of knowledge by areas on the pain management was the highest in the pain characteristics area as mean 13.1/16 and82.1% of right answer. The next assessment and intervention of pain area as 10.6/13 and 81.7% of right answer and the lowest in pain medication area as 6.0/11 and 55.3 of right answer. 3) The mean score of nursing intervention for pain was 2.97/4. 'Holding' was the highest 3.78/4, and next 'Massage(Touching smoothly with the hand)' as 3.58/4. the lowest intervention was 'Music therapy' as 1.87/4 and the next 'use of oral sucorse' 1.88/4. 4) The mean score of barrier of pain nursing was 1.72/4. 'lock of time' was the highest 2.73/4 and next 'absence of prescription' as 2.21/4. The lowest barriers were 'not knowing necessity' as 1.24/4 and 'Thinking of ineffectiveness' as 2.21/4. In conclusion, it was appeared that the degree of knowledge about pain control which the nurse working in NICU has was not high and in the question of the frequency to fulfill pain nursing, they answered that they used it 'from time to time'. The degree of barriers was appeared as 'they had a few barriers' and the main barriers in nursing pain were indicated as exterior factors such as 'they didn't have time to spare', 'they didn't have any prescription' etc. Therefore, I think that the strategy which can improve the degree of knowledge and fulfillment of nurse working at NICU is needed and the devices to decrease or to get rid of the factors which decrease the degree of fulfillment are needed to control pain effectively at NICU.
