    Background: Trauma or excessive exercise, inflammation and degenerative change are important causative factors of shoulder pain. it is required to examine it accurately to make correct diagnosis. Both the ultrasonography and MR arthrography have been widely used to diagnose musculoskeletal diseases. This study was conducted to determine the effectiveness of the MR arthrography and ultrasonography on 67 patient of shoulder pain. Methods: This study was carried out by comparing the result of arthroscopy with a diagnosis based on the rotator cuff tear of patients who were examined using the MR arthrography and ultrasonography, among patients with their shoulder examination performed from June 2002 to June 2004. For 67 patients, their results of the ultrasonography and MR arthrography were compared. Results: The result examined by the MR arthrography and ultrasonography showed that there were high degree of reliability when compared with the results of arthroscophy, while there were advantages and disadvantages in both of them, respectively. Whereas the ultrasonography featured easy and quick examination which may depend on its examiner's experience, the MR arthrography was accurate examination although the patient might have some discomfort. Conclusion: Both ultrasonography and MR arthrography were proven to be useful to evaluate the cause of shoulder pain and expect its prognosis so that they could be used selectively depending on the physical condition of patients. If they are actually applied to the patients with shoulder pain or recurrent dislocation, they can contribute to preventing their further injury and proper treatment.
