Comparative analysis between direct perioperative nursing activities and indirect perioperative nursing activities before and after CCDS implementation : focused on perioperative nursing care to OS patient

This study is to supply the fundamental data for establishment of effective perioperative nursing system by means of comparison and analysis of direct or indirect perioperative nursing activities performed to orthopedic surgery patients before and after Case Cart Delivery System (CCDS), which is one of the flow system. In order to analyze perioperative nursing hours, 15 nurses with more than 9month working experience at orthopedic operating room of 3rd medical institution in Seoul were selected. With explanation of the purpose of this study, their approval was achieved. The data collecting period was from Oct. 13, 2003 to Oct. 17, 2003 and from Apr. 6, 2004 to Apr. 12, 2004 except weekend. Total 178 operating cases were surveyed for this data collecting. Average daily working time is 500min and Considering that operating room entrance time is before 8:00 a.m., extra 30∼60min before regular working time was included for analysis. Scale for perioperative nursing performance used in this study is based on the study of Park, Jung Ho(1995,KDRG) and Kim, Mi Young (2000). Content validity of this method was verified by experts and content validity index of this scale is .97 and cronbachs alpha is .95. These data were analyzed by SAS program using percentage ratio, paired t-test, mean and standard deviation. The result of this study is as follows: 1. As a result of comparing the extent of direct and indirect perioperative nursing performance before and after CCDS, the ratio of direct perioperative nursing performance has changed from 283.28min(53.9%) to 305.52min(58.2%, p=.37) and that of indirect perioperative nursing performance from 46.2% to 41.4%(p=.37). 2. Regarding the daily average hour for the factor of nursing activity in direct nursing category. Scrub activities were in the majority occupying each 172.57min(34.51%) before CCDS, 197.12min(39.42%) after CCDS. The considerable influence of CCDS has been observed in the activity of preparation, connection, handling and separation of operation equipments. 3. Regarding the daily average hour for the factor of nursing activity in indirect nursing category. The observation of operation was in the majority occupying each 46.87min(9.37%) before CCDS, 46.06min(9.21%) after CCDS. The considerable influence of CCDS has been observed in the activity of record keeping, operating stuff preparation, unfoldment of disinfectant package, confirmation of things for CCDS, operating room arrangement and schedule checkup. The above result may confirm that the application of CCSD has considerable influences on some operating hours relating to preparation of operation and suggests that it can increase direct nursing hours. Moreover, based on this kind of study, the continuous verification of the system which enable more effective nursing activity in operating room should be performed.
