    BACKGROUND: Allergy against natural rubber latex is an increasing problem in recent years, especially in health care workers, children with spina bifida. Many studies reported atopic dermatitis could be also one of risk factors of latex allergy. OBJECTIVE: Purposes of this study were not only to access the prevalence of latex sensitization in patients with atopic dermatitis, but to evaluate the relationship between latex sensitization and clinical allergy. METHODS: Of the total of 110 patients, 80 patients were atopic dematitis and 30 patients were controls. A questionaire-based history was taken, skin prick tests (SPTs) with latex glove extract and commercial latex allergen and several commercial fruit extracts were done. If SPT to latex was positive, use test performed. Patch tests for delayed type hypersensitivity were carried out with a small piece of latex glove, commercial latex allergens and rubber additives. RESULTS: Eighteens (22.5%) of 80 atopic dermatitis reported symptoms whenever they used rubber products. In skin prick test, 3 of 80 (3.8%) patients with atopic dermatitis were positive to latex glove extration solutions and also the commercial latex antigens. Reactions to fruit extracts including mixed nuts, potato, tomato, banana were 8.8%, 6.3%, 6.3%, 5.0%, respectively. Use test was done with all of 4 patients positive to SPT, and only one of them showed positive reaction. There is no positive result in latex glove or latex extraction in patch tests. CONCLUSION: There were no differences in latex sensitivities between patients with atopic dermatitis and healthy people. In patch tests, ZnBDC and mercapto mix were the highst rate of reaction addictives of NRLs.

    BACKGROUND: The incidence of cutaneous malignant tumors and surgical approaches to treat them by dermatologists have been increasing recently. OBJECTIVE: The purpose of this study was to analyze our experience of removal and reconstruction surgery to treat malignant skin tumors. METHOD: We clinically analyzed 177 cases of cutaneous malignant tumors which had been operated at the department of dermatology, Chonnam University Hospital in Gwangju during the last 5 years between July 1998 and June 2003. RESULTS: 1. The most common cutaneous malignant tumors was basal cell carcinoma (BCC) (62.7%), followed by squamous cell carcinoma (SCC) (15.3%), malignant melanoma (11.9%), Bowen's disease (5.6%), and extramammary Paget's disease (2.8%), respectively. 2. The most common site of all cutaneous malignant tumors was the face (69.2%), followed by lower extremities (10.4%), upper extremities (6.6%), trunk (6.6%), and scalp (4.9%). In each tumor, the face was the most common site for BCC and SCC, lower extremities for malignant melanoma, trunk and lower extremities for Bowen's disease, respectively. 3. The local flap was the most common method to repair surgical defects (51.1%), followed by primary closure (29.1%), and skin graft (19.8%) in general. In terms of tumor types, the most commonly used method was local flap for BCC and SCC, primary closure for Bowen's disease, and skin graft for malignant melanoma, respectively. In terms of location, the most commonly used method was local flap for the face, primary closure for the scalp and trunk, and skin graft for the upper and lower extremities, respectively.

    BACKGROUND: Skin smoothness in aging skin, in combination of intrinsic aging and photoaging, is of ever-increasing interest which leads to the development of various wrinkle diminuation products including those containing herbal medicines. OBJECTIVES: The purpose of this study was to assess the efficacy of cosmetics containing roots of Ephedra sinica and Betula platyphylla var. japonica on reducing skin furrows. METHOD: Creams containing Ephedra sinica's root and Betula platyphylla var. japonica were applied to forearms of 30 healthy subjects in 2 age groups for 8 weeks and 4 weeks, respectively while silicone skin replicas were taken from medial and lateral sides of the forearm every 4 weeks of the study starting from the week 0. Then the changes of the skin furrows were analyzed with skin visiometer. RESULTS: 1. At week 8, a significant reduction of average roughness and maximum roughness were observed in volunteers younger than age 50 on their Ephedra sinica's root-treated side compared with the placebo-treated side (p

    BACKGROUND: In accordance with the increase of the old age group, the incidence of actinic keratosis (AK) also increases. But there has been no comparative study between the clinical and histopathological margins. OBJECTIVE: The purpose of this study was to compare the clinical margin with histopathological margin of AK in Korean patients. METHOD: All cases of AK confirmed by biopsy from February 1, 1993 to May 31, 2003 were reviewed by hospital records and biopsy slides. Total 50 AK cases excised with 2mm surgical free margin were selected and analyzed clinically and histopathologically. RESULTS: 1. Histopathologically, hypertrophic type of AK was noted in 18 cases (36%), atrophic type in 8 cases (16%), bowenoid type in 5 cases (10%), acantholytic type in 3 cases (6%), pigmented type in 2 cases (4%), and combined form of hypertrophic and atrophic types was noted in 7 cases (14%), combined form of atrophic and pigmented types in 5 cases (10%), and combined form of acantholytic and pigmented types in 2 cases (4%). 2. The total proportion of surgical margin involvement was 44% (22/50): hypertrophic type in 55.5% (10/18), atrophic type in 87.5% (7/8), bowenoid type in 20% (1/5), acantholytic type in 33.3% (1/3), pigmented type in 0% (0/2), combined form of hypertrophic and atrophic types in 28.5% (2/7), combined form of atrophic and pigmented types in 20% (1/5), and combined form of acantholytic and pigmented types in 0% (0/2). CONCLUSION: In 50 cases of AK excised with 2mm surgical free margin, 22 cases (44%) showed tumor cells on the surgical margin. Especially hypertrophic and atrophic types showed significant proportion of marginal involvement. In respect that hypertrophic and atrophic types are more common than other subtypes of AK in Korean patients, skin biopsy and check-up for surgical margin are recommended in all AK.

    BACKGROUND: In recent years alternative medicine such as herbal medicines has been attractive and useful in dermatologic therapy. But there is little scientific information about its therapeutic effects and potential side effects. OBJECTIVE: We performed this study to assess the whitening effect of the root of Ma Huang. METHODS: Thirty subjects were tanned by UVB irradiation on their abdomen and 4 pigmented patches in square were obtained. In a single-blind fashion, participants applied the root extract of Ma Huang to two pigmented patches and vehicle to another patch on the abdomen twice a day, and applied nothing to the other patch for 8 weeks. Participants were followed up every week and checked on a melanin index with a mexameter. We also checked the last clinical improvement 8 weeks after the application of the root extract of Ma Huang. RESULTS: MED of the middle back was 73+/-24.66cmJ/cm2. Melanin index of the abdomen was 473.2+/-16.27. Eight weeks after application, the mean change percent of the melanin index of the right upper and lower squares applied with the root extract of Ma Huang were +0.02%(p=0.96) and -0.16%(p=0.76). The mean change percent of the left upper square applied with vehicle was -0.08%(p=0.89). The mean change percent of the left lower square untreated was +0.20(p=0.65). One-way layout ANOVA showed no difference among four squares. Clinical evaluation of improvement did not satisfy: no change 36%; slight improvement 53%; moderate improvement 9%; marked improvement 2%. CONCLUSIONS: The root of Ma Huang did not show the whitening effect on the UVB-induced pigmentation checked by the mexameter but showed slight improvement in visual assessment.

    BACKGROUND: A number of cytokines, growth factors and inflammatory mediators made by keratinocytes, fibroblasts and potentially other cells in the skin were shown to augment melanin production and/or melanin transfer to keratinocytes. A lot of studies were done to investigate the mechanisms. But there was no studies about sequential relationship between the suggestive factors. OBJECT: To measure the changes in expressions of ET-1, MITF, and PAR-2 after UV exposure on skin in Korean. MATERIAL AND METHOD: We established an immunohistochemical staining protocol for specimens which were obtained from UV-irradiated skin in four healthy Korean males on the 2nd, 4th, 7th days after UV irradiation. RESULT: This study reveals the sequential relationship between ET-1, MITF, and PAR-2. ET-1 was increased at day 2 and decreased at day 4 and 7. MITF increased gradually day 2, day 4, and day 7. PAR-2 increased at day 4 and increased once more at day 7.

    BACKGROUND: The role of stratum corneum has been known to be the major barrier against percutaneous absorption. The change of osmotic gradient onto stratum corneum may affect the permeability barrier function though its mechanism has not been explained. OBJECTIVE: In this study, we have tested hyperosmolar glucose solution (1M, 0.1M) over the living skin or the separated epidermal sheets to determine the penetration-related pharmacokinetics such as absorption, saturation, diffusion kinetics in vivo or in vitro. METHODS: The hyperosmolar glucose patches were applied to 10 healthy volunteers' forearm skin to analyze the absorption profiles through stratum corneum. For investigating the role of osmotic pressure influencing the disposition of glucose, in vitro two compartment model was used to characterize the pharmacokinetics of glucose through epidermal sheets. RESULTS: The quantitative assay of applied hyperosmolar glucose from sequentially stripped stratum corneum of volunteers revealed the high glucose/protein ratio and steep concentration gradient at the uppermost layers down to lower layers. The pharmacokinetic profile of hyperosmolar glucose in vitro shows both the saturation delay pattern and steady flux pattern regarding glucose diffusion. CONCLUSION: The stratum corneum act as a major permeation barrier against glucose disposition, though the concentration-dependent pharmacokinetics by its osmotic gradient were rather different. Thus, the osmolarity-related event over stratum corneum might be a considerable factor during percutaneous absorption.

    BACKGROUND: Androgenetic alopecia (AGA) is one of the most common type of alopecia among all men and women after puberty. The number of patients with AGA appears to be gradually increasing. OBJECTIVES: The purpose of this study was to investigate the clinical status such as age, family history, and type of patients with AGA and to review recent Korean literature. METHODS: 1500 patients with AGA who had visited the Department of Dermatology, Busan Paik Hospital, Inje University School of Medicine during the last 7 years (from May 1997 through December 2003) were examined through the clinical records. RESULTS: 1) There were 1162 male and 338 female patients being most prevalent in the third decade in both sexes. 2) In males AGA, Norwood-Hamilton's type II was the most common. 3) In females AGA, Ludwig's type I was the most common. 4) There was a family history of baldness in 62.9% of relatives in male patients and 48.8% in female patients. 5) The largest number of patients with AGA first visited our department when they had had 1-5 year's history of AGA in both sexes. 6) In the management of male AGA, oral finasteride only or with topical medications was the most common (72.4%) treatment modality. 7) In the management of female AGA, topical minoxidil solution only or with other topical medications was the most common (78.7%) treatment modality. CONCLUSIONS: The proportion of young patients with mild types appears to be high among AGA patients. It might be largely due to psychological stress in social situations.

    BACKGROUND: Communication skill training during medical education, once considered a minor subject, is now ranked as a core clinical skill. OBJECTIVES: The purpose of this study is to evaluate the effectiveness of role play as a method for improving physician-patient communication during the dermatological clerkship. METHODS: Ninety eight senior medical students undergoing dermatological clerkship were included in this study. Initially, the students were briefly lectured on communication skills from a dermatologic professor and two students were randomly allocated for role play. The student in the patient's role was shown the history of the patient with common dermatological problems just before the role play. After the first role play, the patient and physician roles were interchanged and the new student in the patient's role was shown another patient's medical history for the same role play as mentioned above. Two other students were allocated for the third role play with another dermatological problem. A questionnaire survey was used by observing students and the professors to assess the communication skills of the students playing the physician roles in various aspects. After the role plays, the students were allocated to the dermatologic outpatient clinic and with patient consent, they saw real patients as true doctors and the professors evaluated their communication skills using the same questionnaires. RESULTS: As the role plays proceeded, the communication skills of the students were improved. In the outpatient clinic, the students showed better physician-patient communication than ever before. The improvement of communication skills in the outpatient clinic was clearly shown both in the students who had observed the role plays and in those who had performed the physicians' or patients'roles. CONCLUSION: Communication skills in medical students undergoing dermatologic clerkship can be effectively trained, and improved by use of role play.

    The diabetic thick skin syndrome is a common disease in the children of diabetes mellitus. But, it has not been reported in Korea. It is characterized by the stiff joints of hands and the indurated skin of both hands and arms. The simple screening test is prayer sign that patient is unable to bring fingers together because of contractures of proximal and distal interphalangeal joints. We present an 11-year-old Korean child with stiff joints of both hands and waxy, indurated skin of both hands and forearms that had been increasing in extent for the last 4 years.
