    초록

    Most of people receive high-quality health care services. However, too often, patients receive excessive services that undermine the Quality of care and needlessly increase costs. At other times, they do not receive services that have proven to be effective at improving health outcomes and reducing costs This study reviewed some of the quality problems including underuse, overuse, misuse, and variation in use of health care services. The study also addressed some approaches to improve the ability to measure and report on Quality of care being delivered. Some effective strategies have been developed to improve the quality of health care delivered to patients every day. The promising developments are described in this study, along with other efforts by private sectors, health care facilities, corporations and the labor unions. However, it is also clear that a patchwork of efforts will not lead to significant, continuous nationwide improvements in health care quality.

    초록

    The aim of this study sets out to discover a desirable form of public health of mental programs among the alternative ones. Especially, this study attempts ; (ⅰ) to investigate the factors that affect the performance of public health of mental programs ; (ⅱ) carry out cost-effectiveness analysis (CEA) for the various type of public health of mental programs ; (ⅲ) identify innovative strategies for efficiency of public health of mental programs. Cost-effectiveness analysis is used to compare the performance of all the public health of mental hospital programs. Measure of cost is used to health programs cost from 1992 to 2001. The index of effectiveness is used to adjusted bed turnover rounds and adjusted patient days per specialist of all the mental hospital programs from 1992 to 200l. As a result of these analyses, policy options as follows are recommended ; (ⅰ) public mental hospital programs should be enough to provide health services adequately as open system ; (ⅱ) facilities and equipments of public mental hospital programs should be provided sufficiently. (ⅲ) the efficiency of mental hospital programs should be raised by active management for containment of hospital expenditure, economics of scale, change rate of mental inpatient. Ultimately mental hospital programs in public sector are to be fostered for the promotion of health care by enhancing the equity and efficiency of health services.

    초록

    Asymmetric distribution of health care resources in South Korea has been one of the most important concerns in health care. Especially, the shortage of health care resources in rural areas is the top priority for health policymakers to handle. Relevant to this, it is essential to define and optimize the role and function of public health care organizations. Important questions raised are equitable accessibility, availability and efficient delivery of health care services. There are 17 Health Center County Hospitals(HCCHs) amongst public health care organizations. Unlike Health Centers (HCs), which are unable to provide inpatient care beds, HCCHs provide these services. Nowadays the gradually decreasing number of rural community inpatients has been a challenge to HCCHs. They need to seek changes in their role and function consecutively. This study aims for two objectives. One is to set the appropriate direction of health center county hospitals and the other is to find out strategies for efficient management to improve community people"s health in the near future. Data were collected by the method of self-administrative questionnaires developed by the researcher. The sample cases are 1,207 communities" residents in J county. This study shows firstly, the major disease in communities" residents is chronic degenerative disease, i.e. arthritis, high blood pressure and diabetes mellitus. Secondly, the utilization rate of HCCHs is low and that of the inpatient care services is the lowest. And lastly, communities" residents want HCCHs to be reorganized for the provision of physical therapy(82.6%), emergency care(88.8%) and oriental care services(81.4%) as health care service function as well as inoculations(87.1%), health activities for the elderly(83.6%), and home health care(75.8%) as health activity function. In conclusion, the inefficient bed management policy and the poor performance of health activities of HCCHs have been major problems because HCCHs have implemented the inpatient care service function and original health activity function dichotomously. Thus it is desirable, from a strategic perspective, to decrease the current number of beds down to the minimum size necessary and reinforce emergency service and health promotion activities. Concurrently, the reallocation of health personnel from inpatient medical care to health activities, which is sought by community residents, could be a way to increase efficiency in health care resource allocation. In respective of managerial efficiency, these are the sensible steps to take in order to improve the functions of HCCHs at present.

    초록

    The purpose of this study is to analyze the status of female doctors in Korean health institutions. This study first reviews the status of female doctor, particularly Korean female doctors historically. And then, this study investigates how is the status of Korean female doctor through statistics. Also, the socio-economic status of female doctors is compared with that of male doctors through surveys. The data were collected in general hospitals and university hospitals, Pusan and Seoul. In survey, 311 doctors reported the questionnaires. Several crucial findings in this study regarding the status of female doctors in Korean health institution are as follows. First, the status of female doctors is lower than that of male doctors in Korean health institution now and the past days. Second, Gender typing is prominent in departments, especially in surgery, and in hospital directorships. Third, Male doctors hold higher positions such as chiefs of staff (4.3 times), hospital vice directorships, and faculty chiefs, among others. Fourth, Female doctors in hospitals receive relatively lower incomes compared with male doctors, even though a larger proportion of them work in hospitals.

    초록

    The purpose of this study was to examine the general distribution on cancer screening behaviors and to find out the factors associated with stomach, breast, and cervical cancer screening behaviors by stages of change. 1,067 women whose ages ranged from 31 to 60 were selected by a random cluster sampling method, and subjects were answered by self-reported questionnaires; this survey was performed between January, 2000. We used the TTM(Transtheoretical Model) to classify the screening behavior by stages of change. Stages of change were 5 steps; Precontemplation(PC), Contemplation (CP), Preparation(PR), Action(AC), and Maintenance(MT). And the multiple regression analysis was performed to derive the significant variables from demographic factors and health promotion behaviors. All analyses were performed by the PC-SAS 6.12. The major results were as follows : 1. The highest distribution stage on cancer screening behaviors by stages of change was that stomach and breast cancer screening behaviors were contemplation(56.2%, 54.3%) and cervical cancer screening behavior was maintenance(20.4%). 2. As to the factors associated with stomach, breast, and cervical cancer screening behaviors, there were significantly for age, educational level, income, occupation, married status, smoking, drinking, exercise, eating habit, sleeping time, visiting doctor, and health promotion behaviors scores (P<0.05, P<0.01). 3. It showed that the predictors on stomach cancer screening behavior by stages of change were age, breast cancer screening behavior, and cervical cancer screening behavior(P<0.01). However, the predictors on breast cancer screening behavior by stages of change were educational level, health promotion behaviors scores, stomach cancer screening behavior, and cervical cancer screening behavior(P<0.01). And the predictors on cervical cancer screening behavior by stages of change were age, occupation, married status, health promotion behaviors scores, stomach cancer screening behavior, and breast cancer screening behavior(P<0.01).

    초록

    This study analysed the relationship between socioeconomic status indexes, such as education, income, wealth and subjective economic status, and chronic diseases prevalence. The study used the data of 445 gentlemen and 710 ladies age 65 and over from KLIPS. To estimate relative risk (odds ratio) of chronic diseases prevalence, a multiple logit regression was conducted. To get the net effect of socioeconomic factors on the chronic diseases prevalence, we added demographic and health behavior variables in the model as control variables. We analysed various models which include ⅰ) demographic variables, such as age, absence of spouse, city, and family type, ⅱ) health behavior variables, such as (ir)regular exercise, smoking, and drinking, and ⅲ) socioeconomic-status(SES) variable, such as income, education, financial asset, own-house, own real estate, and self-reporting economic status. After controlling the effects of demographic and health behavior variables, we found substantial socioeconomic inequalities in chronic diseases prevalence and a stronger effect for education than for income and wealth for both gentlemen and ladies. The study results suggest that health programs such as health education are needed for the lower SES people, and not only for aged people but for middle aged people to reduce health inequality.

    초록

    This is an effort to examine effects of religiosity on health behavior among high school students. Data were collected from a convenience sample consisting of high school students of 565 through a self-administered questionnaire survey method. Religiosity was measured by a variety of religious activities. Health behaviors included health-related behaviors identified in the study of Alameda county. Data revealed that patterns of health behaviors varied among students across three types of high schools, including prep, vocational, and mission schools. Practices of health behaviors were associated with subjective health status, being loved, monthly monetary allowance, religiosity, the number of supporters, a feeling of satisfaction for the school life. Multiple regression analyses by a stepwise method identified the model in which religiosity has effects on health behavior practices. The effects of religiosity on health behaviors were not found in male students.

    The Purpose of this study was to examine the subjective quality of life in the rural residents on west areas of Chungnam province. The subjects were 1,226 persons on west areas of Chungnam province. Survey was conducted from July 12 to July 17, 2003, and the collected data were encoded and analyzed with SPSS Window 10.0 program. The measurement instrument was Korean WHOQOL-BREF(World Health Organization Quality of Life Assessment Instrument) which was consisted in the physical health, psychological health, social relationships, and living environment domain. Data were interpreted by frequency analysis, mean, standard deviation, chi-square test, t-test, ANOVA test, Duncan post Hoc test, and multiple regression analysis. The major findings may be summarized as follows; 1. Monthly earns were very statistically significant difference in the all domain of quality of life(p<0.001). Also, sex, age, education and occupation were significantly significant. 2. Peoples living with patient were lower quality of life than the others. And the lower family sizes have, the higher social relationships in quality of life have. 3. Period of residence was statistically significant. difference in the social relationships of quality of life. Also, Residents living industrial complex area were lower life environment domain in quality of life than residents dwelling house area. Consequently we will make an effort on improvement of quality of life in the rural residents. We found out that economic status is important factor in quality of life, so we will make opportunity to raise the farmhouse wifes" income. Also Peoples living with patient were lower quality of life than the others, we will effort to improvement of health of rural residents.
